Assessment priorities
- Determine duration, stool pattern, and severity impact on function.
- Review medications, hydration, diet, and mobility contributors.
- Screen for alarm symptoms suggesting secondary pathology.
Red flags
- Severe abdominal pain, vomiting, bleeding, or weight loss.
- New-onset constipation in higher-risk age group with alarm features.
- Progressive symptoms with inability to tolerate oral intake.
Initial management
- Start stepwise bowel regimen with hydration and fiber strategy as appropriate.
- Deprescribe or adjust constipating medications when feasible.
- Set realistic response timeline and escalation criteria.
Follow-up and escalation
- Reassess if response is incomplete or alarm symptoms emerge.
- Escalate for obstruction concern, severe pain, or persistent bleeding.
- Coordinate specialist input for refractory or complicated constipation.
