Assessment priorities

  • Determine duration, stool pattern, and severity impact on function.
  • Review medications, hydration, diet, and mobility contributors.
  • Screen for alarm symptoms suggesting secondary pathology.

Red flags

  • Severe abdominal pain, vomiting, bleeding, or weight loss.
  • New-onset constipation in higher-risk age group with alarm features.
  • Progressive symptoms with inability to tolerate oral intake.

Initial management

  • Start stepwise bowel regimen with hydration and fiber strategy as appropriate.
  • Deprescribe or adjust constipating medications when feasible.
  • Set realistic response timeline and escalation criteria.

Follow-up and escalation

  • Reassess if response is incomplete or alarm symptoms emerge.
  • Escalate for obstruction concern, severe pain, or persistent bleeding.
  • Coordinate specialist input for refractory or complicated constipation.
