Minimum pre-referral workup

  • Provide renal function, urine ACR, blood pressure, weight trend, and cardiovascular comorbidity context.

Urgency tier

  • Urgent escalation for severe recurrent hypoglycemia, ketosis concern, or marked symptomatic hyperglycemia.

  • Routine referral for optimization of complex regimens or difficult treatment decisions.
